Job Description:

We are looking for a Physical Therapist to provide one-on-one therapy care for patients in their home setting.

Primary Job Responsibilities:

• Assess a person's condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence.
• Collaborate with interdisciplinary team and colleagues to ensure a person's needs are met.
• Demonstrate ability to perform standard therapy measurement techniques, including ROM, strength, and balance tests.
• Utilize a variety of therapeutic modalities to maximize care provided to clients

Requirements:

• Vermont PT licensure
• Home Health experience and OASIS experience a must, as this position will case manage
• McKesson/Netsmart EMR experience required
• COVID VACCINATION REQUIRED (CMS guidelines). No Exemptions accepted.
