HealthCare Support is actively seeking a Physical Therapistto fill an opening with a great healthcare organization covering two boroughs:Queens and Brooklyn, New York.
Daily Responsibilities for Physical Therapist:
- Performing physical therapy in patient homes
- Work alongside the Skilled Nursing team, and with physicians, to assess the individual rehabilitative needs and develop a plan of care for the patient
- Complete documentation accurately and submits to the agency office in a timely manner
- Work post-surgical conditions like hip and knee replacements, diseases such as Parkinson's and Multiple Sclerosis, and events such as a stroke, heart attack, or fractures after a trauma.
Required Qualifications for Physical Therapist:
- Valid Driver's License and auto insurance
- CPR Certification
- Current PT License
- 1+ years of experience as a Physical Therapist
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist
Benefits for Physical Therapist:
- Eligible for insurance after 60 days of employment and PTO accrual after 90 days

Hours for thisRN Position:
- Monday-Friday 8 am-5pm
Pay Details: $90-100K (Dependent on experience)
- $5K SOB paid out during 1styear + $3K retention each year(starts at end of 2ndyear)
