Position Title: Physical Therapist (Adult Home Health Setting)

We are currently seeking dedicated Physical Therapists to make a significant impact in adult home health settings across metropolitan Houston.

**Primary Duties:**

**Assessment and Diagnosis:** Conduct thorough assessments to identify physical therapy needs and craft individualized treatment plans for adult patients in home health settings.

**Tailored Treatment Plans:** Develop and execute personalized treatment plans designed to address the unique goals and requirements of adult patients, with a focus on enhancing mobility, strength, and overall function.

**Therapeutic Interventions:** Administer hands-on therapy and therapeutic techniques to facilitate improvements in mobility, coordination, and activities of daily living for patients.

**Collaboration:** Work closely with interdisciplinary teams, including physicians, nurses, and caregivers, to ensure comprehensive and coordinated care for adult patients.

**Progress Monitoring:** Continuously evaluate patient progress and adapt treatment strategies as necessary to optimize functional outcomes and achieve therapy objectives.

**Patient Education:** Educate patients and their families on effective therapeutic exercises, techniques, and strategies to support ongoing progress and independence.

**Documentation:** Maintain accurate and detailed records of patient assessments, treatment plans, progress notes, and outcomes in adherence to regulatory standards.

**Compliance:** Ensure that all therapy services and documentation comply with state and federal regulations, as well as the policies and procedures of the home health agency.

**Education:**

- Master’s or Doctorate degree in Physical Therapy from an accredited college or university.

**Licensure/Certification:**

- Texas State License in Physical Therapy

- Certification from the National Board of Certification in Physical Therapy (NBCPT)

- CPR Certification

- Professional Liability Insurance

**Other Qualifications:**

- Prior experience in adult home health settings is preferred.

- Strong interpersonal and communication abilities.

- Ability to work autonomously as well as part of a collaborative team.

- Completion of TB testing/screening.

Job Type: Part-time

Salary: $70.00 - $80.00 per hour

Expected hours: 2 – 3 per week
