Company's clients are mentally- ill and may be direct post-operative; therefore, contractor must be able to show two years of experience in this area. Contractor is expected to provide an estimated eighty (40) hours per week of physical therapy services during the work week from Monday – Friday. Hours may fluctuate and are subject to a two (2) hour minimum charge per site visit. The New York State licensed Physical Therapist shall:
A) Act as physical therapy consultant.
B) Assess and evaluate client’s condition to develop a physical therapy treatment plan based upon physician’s request.
C) Review client’s treatment goals, treatment, and medical history.
D) Perform and interpret standard muscle, nerve, joint and functional ability tests.
E) Administer appropriate physical therapy treatments when indicated.
F) Prepare documentation of assessment, evaluation, delivery of treatment services and patient progress for the client’s medical record.
G) Consult with client’s treatment team as needed.
H) Be supervised by Company’s Director of Medicine.
Hours and Documentation:
A) Contractor must be flexible in scheduling to be available to meet the needs of the patients . Also, in various out-patient clinics as needed / approved by the Medical Director.
B) Contractor must be able to provide coverage in the event primary Physical Therapists are unavailable due to illness, vacation, etc.
Qualifications, Licenses and Permits:
A) Contractor must supply current New York State licensed Physical Therapists
B) Designated staff must possess, at no cost to the STATE, all qualifications, licenses, and permits to perform physical therapy services; contractor must engage in the business of physical therapy consultant services that may be required within the jurisdiction (state & local) where the specified service is to be performed, comply with all state and local regulations governing physical therapy consultant services.
C) Contractor must have at least four (4) years satisfactory experience in providing physical therapy consultant services to mentally ill clients who may be directly post-operative. (Due to the nature of their mental illness, many rehabilitation centers will not accept our clients for treatment.)
D) Contractor shall be required to submit a list of at least four (4) references for whom contractor has provided like or similar services within the past five years.
E) Contractor is expected to supply only qualified and experienced staff for this contract.
