Qualifications:
Master's degree with New York State license and certification is required. Experience in hospital/clinic/long term care setting and needs of visually impaired population, preferred.
Accountability:
The PT is responsible to the Program Director.
Responsibilities:
- Evaluate, plan, develop, and implement physical therapy services as ordered.
- Assure that therapy personnel and registrants follow established policies and procedures at all times.
- Review physical therapy policies as requested, and participate in making recommendations for changes.
- Maintain a working relationship with registrant's community physician.
- Review requests for physical therapy and physician's orders. Interview and evaluate registrant to determine type of treatment and schedule therapy.
- Participate in development and implementation of registrant care plans, registrant assessments and discharge planning, etc., as necessary.
- Comply with established safety precautions at all times.
- Complete necessary forms, reports, etc., and submit to the Program Directoras required.
- Maintain treatment records and progress notes as required per GuildCare policy.
- Recommend to the Program Directorthe equipment and supply needs of the department.
- Assure that the therapy area is maintained in a clean and safe manner for registrant comfort and convenience by assuring that necessary equipment and supplies are maintained to perform such duties/services.
- Assure that all therapist notes are informative and descriptive of the care provided and of the registrant's response to the care.
- Complete service logs accurately and in a timely manner.
- Observes and implements the Registrants' Bill of Rights.
Physical demands:
- Standing for long periods, walking, lifting/transferring registrants, using lifting equipment when required.
- Bending, stooping, squatting, twisting, reaching above and below shoulder level.
- Able to push/pull registrants in wheelchairs.
- Able to lift and carry equipment and/or supplies
