The Population Health Service Organization (PHSO) Integrated Care Management Department is hiring for a Health and Wellness Coach to join their team!
- The Health and Wellness Coach will work primarily with patients in need of a joint replacement for short-term coaching. Patient's targeted goals focus on weight loss, smoking cessation and behavioral modification.
- This is a hybrid position - our Health and Wellness Coach will be working remotely and/or at Integrated Care Management or Tilly Drive to see patients in person.

The UVMHN (University of Vermont Health Network) PHSO (Population Health Services Organization) is a physician and payer partnered operation that provides the right constellation of services (e.g. care management and care coordination, data and analytics, provider reporting, Quality, Risk Adjustment, and Value-based Contract Performance, etc.), that enables the health network to successfully perform in High Value Care and Value-Base Contracts.
JOB DESCRIPTION:
The PHSO Health & Wellness Coach is responsible for engaging identified populations in behavior change through motivational interviewing, behavior modification techniques, and building trusting relationships. The incumbent will serve selected patient populations across to promote the Health and Wellness to achieve optimal patient-centered outcomes.
EDUCATION:
Associates Degree in Human Services
or
2-3 years of experience in a healthcare setting (or equivalent)
Bachelor’s level degree in Human Services or other health field preferred. The National Board Certification credential (NBC-HWC) or equivalent preferred.
EXPERIENCE:
Health Coach experience preferred.
Care Coordination experience preferred.
Customer Service experience preferred.
Experience in motivational interviewing/stages of change is highly desirable.
