Patient Engagement Advisors (PEA) is seeking a full time Transition Specialist for Transitions of Care ("Care 360") program at AdventHealth Tampa.

As a Transition Specialist, your role is to set patients up for success to transition out of the hospital into their next stage of care.
Here are some of the tasks you will perform to assist in this transition:
- Schedule patient’s follow-up appointments
- Schedule patient’s transportation as needed
- Organize meal delivery specific to patient’s condition
- Deliver discharge prescription medications
- Assist in value / savings programs for patients as appropriate
- Other services that generally help patients with adherence / compliance so they can see improvements in their overall health outcomes
*Note: all Transition Specialist deliveries occur onsite to patient rooms within the hospital.

Requirements:
- High School Education or equivalent required
- 6 months Pharmacy Technician or 1-year medical assistant / patient care technician / patient care coordinator / clinic front desk associate / nursing assistant / other healthcare or medical experience required
- Retail Pharmacy Technician experience preferred
- Other related healthcare, hospital, or medical experience will be considered
- Ability to report to work at AdventHealth Tampa as scheduled; ability to be on feet – standing, walking, and/or climbing stairs 8-10 hours daily
- Ability to receive required vaccinations (as required with PEA or hospital) unless approved for exemption
COVID-19 Vaccination Expectations
PEA requires onsite Transition Specialists to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, UNLESS they are approved for a reasonable accommodation based on disability, medical condition, pregnancy, or religious belief that prevents them from being vaccinated.

You must be fully vaccinated (per CDC guidelines) and provide vaccination documentation prior to your first day of employment.
If you are unvaccinated (due to above listed reasons) these requirements will apply:
You will be required to complete the vaccine exemption request form within 48 hours of receiving a job offer based on a religious or medical reason. Additionally, you will be required to receive approval from PEA prior to your first day of employment, that your exemption request has been approved. As a part of this process, you may be required to provide information or documentation supporting your request for exemption.
**PEA may decline to move forward with the hiring process or terminate employment, as allowed by law, if candidate / new hire does not: meet proof of vaccination / accommodation request (including reason verification or supporting documentation as allowed by law) deadlines or if the request for an accommodation is not approved.
