Job Title: Pharmacists
LOCATION: Ventura, CA
Hours of Operation: · Monday to Friday: 0800-1700 (with a daily break of 60 minutes)· Saturday: 0900-1300· Total of 44 hours per week, with flexibility for emergent situations
QUALIFICATIONS: 1. Bachelor of Pharmacy (PharmD) degree from an accredited institution.2. Valid license to practice pharmacy in the state of California.3. Minimum of three (3) years of experience as a licensed pharmacist, preferably in a mental health or community pharmacy setting.4. Demonstrated understanding of psychiatric pharmacotherapy principles, including knowledge of psychotropic medications, side effects, and drug interactions.5. Strong interpersonal and communication skills, with the ability to effectively engage with clients from diverse cultural and linguistic backgrounds.6. Experience working collaboratively in multidisciplinary teams to provide patient-centered care.7. Familiarity with electronic health record systems and proficiency in documentation and reporting.8. Flexibility to work non-traditional hours and travel to various community locations throughout Ventura County.9. Commitment to cultural competence, equity, and social justice in healthcare delivery.10. Ability to maintain a positive working relationship with county agencies, community partners, and other stakeholders.
RESPONSIBILITIES: · Medication Management:Dispense psychiatric medications to clients in adherence with prescribing guidelines and regulations.Conduct medication reconciliation and review for clients receiving mobile mental health services.Monitor medication effectiveness and side effects, adjusting treatment plans as necessary in collaboration with the multidisciplinary team.Provide education to clients and their families about medication usage, potential side effects, and adherence strategies.· Patient-Centered Care:Collaborate with the multidisciplinary team to develop individualized care plans that address the unique needs and preferences of each client.Engage clients in shared decision-making regarding their treatment goals and preferences.Conduct medication therapy management sessions to optimize therapeutic outcomes and promote patient empowerment.· Community Engagement:Participate in pop-up clinics and outreach events in vulnerable communities to provide medication-related education and support.Establish rapport and trust with clients and community members through culturally competent communication and interaction.Work collaboratively with community partners, including local hospitals, human services agencies, and homeless shelters, to ensure coordinated care and support for clients.Documentation and Reporting:Maintain accurate and up-to-date records of medication dispensing, interventions, and client interactions in the Electronic Healthcare Record (EHR).Generate monthly reports detailing medication-related activities, client engagement metrics, and outcomes data.Track and report assessments, injections, appointments, and other relevant data to meet administrative requirements and support program evaluation.· Continuous Improvement:Stay informed about current developments in psychiatric pharmacotherapy and evidence-based practices in mental health care.Participate in ongoing training and professional development activities to enhance knowledge and skills in delivering mobile mental health services.Collaborate with the multidisciplinary team and program stakeholders to identify opportunities for program improvement and innovation.
