Qualification Requirements:
- Valid and active license issued by the California Board of Pharmacy
- Proficiency in the English language (Speaking/Writing)
Responsibilities:
The Pharmacist will provide services under the direction of the Pharmacist-in-Charge and/or the Institutions CEO/CME or designee, within the scope of practice. The pharmacist's duties include, but are not limited to:
- Review prescriber Rx for accuracy, completeness, authenticity, and legality.
- Assess Rx against patient/youth profile for drug errors, interactions, and delivery issues.
- Verify prescription compliance with the Formulary and forward non-formulary requests for approval.
- Prepare, compound, and dispense drugs and pharmaceutical preparations as needed.
- Perform computerized order entry and type prescription labels.
- Fill prescription/medication orders and prepare or verify l.V. Admixtures.
- Consult with medical staff and the Review Committee, including discharge consultations.
- Maintain records of drugs, poisons, and narcotics as required by Federal and State Laws.
- Keep records of received, on-hand, and dispensed drugs, poisons, and chemicals.
- Implement and maintain medication delivery systems following Systemwide P&T Committee directives.
- Assist in estimating drug requisition needs as required by the Pharmacist-in-Charge.
- Supervise and instruct Pharmacy Technicians in standard and routine tasks.
- Perform other assigned duties.
