Millennium Physicians pharmacy is currently looking for a pharmacist with a clinical and/or specialty background who is used to working directly with physicians, other health professionals, and patients to ensure that the medications prescribed for patients contribute to the best possible health outcomes. This position calls for a pharmacist who is well-rounded that has past retail and a clinical background and can work health care settings where they have frequent and regular interactions with physicians and other health professionals, contributing to better coordination of care.
General Summary of Duties: The pharmacist will be responsible for the provision and oversight of pharmacotherapy for oral oncology patients in the ambulatory care setting.
Essential Functions:
- Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices
- Demonstrates performance improvement in the following: clinical care/outcomes, customer/service improvement, operational system/process, and safety
- Demonstrates/models Millennium’s standards of performance in interactions with all customers (internal and external)
- Complies with departmental policies regarding issues of employee, patient, and environmental safety and follows appropriate reporting requirements
- Maintains confidentiality of all materials handled within the company as well as the proper release of information
- Evaluate the appropriateness and effectiveness of the patient’s medications
- Recognize untreated health problems that could be improved or resolved with appropriate medication therapy
- Follow the patient’s progress to determine the effects of the patient’s medications on his or her health
- Consult with the patient’s physicians and other health care providers in selecting the medication therapy that best meets the patient’s needs and contributes effectively to the overall therapy goals
- Advise the patient on how to best take his or her medications
- Support the health care team’s efforts to educate the patient on other important steps to improve or maintain health, such as exercise, diet, and preventive steps like immunization
- Refer the patient to his or her physician or other health professionals to address specific health, wellness, or social services concerns as they arise
- Participates in order entry and related monitoring activities, including:
- Allergy and allergy history clarifications
- Medication history
- Assist physician’s with drug selection when requested
- Interactions including drug/drug, drug/food, drug/disease state
- Pain management
- Pharmaco-economics
Experience:
- A doctorate degree in Pharmacy from an accredited College of Pharmacy. Requires current and unrestricted pharmacy licensure in the state of Texas.
- At least 5 years’ experience required, previous PIC or manager experience preferred
- Must possess good organizational, problem solving and computer skills.
- Must possess good written and verbal communication skills for interfacing with all levels of staff, Physicians and other contacts. A plus if a knowledge of Computer Rx software, particularly pharmacy system software, Windows applications
- A background in oncology is preferred
