We are currently recruiting for an individual who is self-motivated and passionate with a can-do approach, to work as an area supervisor over a small group of caregivers and clients. We are looking for someone to work on the east side of the Denver metro area from Aurora to Thornton. Job Summary:

Provides coordination and support for all assigned client services. Ensuring client receives all authorized services. Provides care to assigned clients when back-up coverage is needed. Conducts training and appropriate follow up to ensure that PCW performs required services. Coordinates with IHSS-RN to ensure Skills Validation are completed and documented. Audits and supervises PCW(s) to ensure care services, client files, records, new client “on-boarding”, client supplies, coordinating client transportation and appointments/activities are performed as authorized and to the company standards. Obtaining and reviewing client service notes Including maintaining computerized client master service plan, IHSS Care Plan, PAR/Authorizations, Electronic Service Notes Reviews and updates, Observation and Supervisor visits documentation, tracking and follow-up. Documents follow up and escalate all client complaints, and incidents.

Must have open availability Sunday through Thursday and be able to work one weekend per month (Friday and Saturday) of on-call that is mandatory for all Supervisors (additional compensation is provided for the on-call weekend). Our clients needs vary and some receive services as early as 7:00 AM and as late as 11:00 PM, 7 days per week. This is a great opportunity for someone who is seeking an entry level management position in the home care/home health environment. Must have own automobile and no barriers to full availability such as childcare issues and a second job/"gig work".

Responsibilities/essential functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.

1. Direct clerical support services for the agency team/area assigned.

2. Provide staffing support for caregiver vacancies ensuring clients receive services.

3. Provide direct on-site assistance and training support for caregivers.

4. Provide coordination and support for new client “on-boarding”.

5. Communicate necessary information to Agency team to ensure coordination of services and activities.

6. Perform job in compliance with agency policies and procedures as well as community and professional standards.

Qualifications/Educational Requirements:

1. Active CNA Certification or Medical Assistant is strongly preferred

2. High School Education, college preferred

3. Clear background and pass a drug test for illegal substances

4. Computer skills.

5. Excellent written and verbal communication skills. Speak, read, write and comprehend English.

6. Valid driver's license and vehicle is required

7. Excellent organizational and time management skills.

8. Ability to do at least one on call weekend a month is required
