Education/Training/Experience:
- High School Diploma or GED equivalent.
- Current certification as a Nurse Aide with the Texas Department of Health( preferred); and documented evidence of status on the Texas Department of Health Nurse Aide Registry with no findings of service recipient abuse or neglect, or misappropriation of service recipient property.
- At least one (1) year’s experience in direct care of service recipients in an institutional setting.
- At least one (1) year of full-time experience within the last five (5) years in direct patient care in a home health or institutional setting.
- Fluent in English and Spanish (read, write, and speak) desirable.
- Staff will be required to submit proof of their vaccination status or may request to be exempt from vaccination due to a religious and/or medical accommodation.
- Certificate or license, as applicable, by the appropriate State agency as a qualified practitioner.
- State of Texas vehicle operator’s license and proof of liability insurance.
Essential Functions:
- Ensures that clients have access, as needed, to all five pillars of the social determinants of health with a trauma informed lens through Agency services and/or community partners.
- Provide quality care in a respectful, sensitive, supportive, and timely manner.
- Review and follow the Individualized Service Plan developed by the Service Coordinator or designee.
- Provide service recipients with in-home care/home maintenance, meal planning/preparation, correspondence and errands, and other tasks as agreed upon by service recipient and Service Coordinator as documented in the Individualized Service Plan.
- Assist service recipients to maintain a clear, safe, and comfortable environment, and perform duties as indicated by the service recipient’s Individualized Service Plan
- Report any changes in service recipient status or condition immediately to the Service Coordinator or Manager, as appropriate.
- Accurately complete and submit, in a timely manner, all required reports and documentation, to include, but not limited to, service delivery records, timesheets, mileage records, and incident reports.
- Maintain service recipient and family member confidentiality in accordance with agency standards.
- Attend agency, program and staff/team meetings, in-service and educational trainings/meetings, and other meetings as assigned.
- Maintain good relationships with stakeholders (including current and potential clients, community and co-workers), focusing on providing excellent and culturally sensitive customer service and presenting a positive image of the agency in all community and stakeholder interactions.
- Maintain a working environment which encourages mutual respect, promotes respectful and congenial relationships between employees and is free from all forms of harassment.
- Perform other duties as assigned, including presentations to community groups and organizations, in accordance with the agency’s “team” philosophy.
