Per Diem Social Worker LMSW - Discharge Planning Weekends - Weill Cornell Days

The Per Diem Social Worker plays an important role in continuity of care and patient flow . They work to expedite patient’s discharge from the hospital on weekends to complete the patient’s episode of care. The Per Diem Social Worker is able to mitigate weekend family or resource barriers to discharge, they work collaboratively with the interdisciplinary team and post-acute care partners to execute discharge plans. They also conduct assessments and work with patients and families to help set discharge expectations and provide education to newly admitted patients.

This position requires 4 weekend shifts/month.

This is a day shift position 9am-5pm.

Preferred Criteria

• Experience as a Float Social Worker

Required Criteria

• Master’s Degree in Social Work (MSW)

• New York State License as a Licensed Masters Social Worker (LMSW)

• At least one year of inpatient Medical/Surgical experience
