Position: Licensed Social Worker Foster Care - Per Diem
In this role, you'll work up to 25 hours a week providing crucial support to families and children in need. Full Job Description
Job Title: Licensed SW Foster Care- Per Diem
Department: Foster Care
Title of Immediate Supervisor: Program Director
Position Summary: Provides a maximum of 25 hours a week of social work support to include family and individual services. Works well in an interdisciplinary treatment team including psychiatry, health service, educational, and paraprofessional staff. Maintains all record keeping requirements, correspondence, and documentation needed to conduct the most professional coordination of care.

Responsibilities/Duties:
1. Completes all FASP’s , 29-i assessments, Connections notes, Treatment Plans, Adoption paperwork.
2. Ensures that the services being provided are responsive to the family vision and needs identified in the FASP and Treatment
Plan and that the family is experiencing voice, access, and ownership of their plans and services.
3. Works virtually in tandem with an in-person service provider to complete sessions, assessments, and or consultations.
4. Seeks community resources for the family, with the assistance of the child and family team, and refers families and clients to
community resources and other organizations.
5. Seeks to empower families to manage their own lives post discharge. Teaching the family the skills to marshal resources on
their own and advocating for themselves when necessary.
6. Will maintain and manage case records in accordance with the Department of Social Services and Family Court guidelines as well as 29-i regulations.
7. Will maintain timeliness and quality of written reports and records.
8. Will provide treatment as indicated in the Service plan. This may include family, individual work.
9. Provides reports, assessments, petitions, and recommendations to referral agencies, including Family Courts. Attend court hearings when court is held virtually as available.
10. Will take a learning stance relative to professional development, including utilizing clinical supervision, reading and training.
11. Will serve as a liaison and ambassador for the agency and the community.
12. Will serve as an advocate for families and youth consistent with the ethics of social work.
13. Attends team meetings when requested to coordinate services for family/youth.
14. Assists family with attending or receiving accurate information regarding psychiatric appointments, follow-up, and medication recommendations.
15. Attends Social Work Department meetings as well as own supervisory sessions when requested to do so.
16. Facilitates referrals for on-going services (aftercare) as necessary and provides these services as appropriate.
17. Ensures that departmental and agency policies are followed in the course of their duties.
18. Maintains all mandatory in-service training.
19. All other duties as assigned by the immediate supervisor.

Minimum Education and/or Professional Qualifications/Skills
● LMSW Social Work or related field, and one year of experience working with youth/families at risk. (required)
● Experience in the provision of individual, family, and group services to adolescents and their families.
● Experience in/commitment to development of community resources and informal supports designed to meet the
needs of families within their own cultural and community context.

8. Must comply with Agency safety standards and be responsible for own actions and conduct concerning safety and healthy
working conditions.
