REGISTERED NURSE (RN) DUTIES/RESPONSIBILITIES:
- Responsible for the coordination, development and direct supervision of the agency’s paraprofessional staff
- Provides in-field professional supervision and on-the-job training to personal care aides
- Assists with teaching Personal Care Aide training classes
- Provides medical oversight for clients in their homes- including but not limited to assessments, obtaining MD orders, monthly contacts and maintaining an appropriate Plan of Care.
- Answer all phone calls received on behalf of the agency, and follow up according to agency policies and procedures
- Educate employed caregivers to provide care for patients as indicated.
- Maintain regular communication with agency staff and the patient’s referring physician and/or case manager concerning patient/family care needs and response to interventions.
- Maintain up-to-date patient records so that problems, goals and interventions are accurately and clearly stated and changes are reflected as they occur.
- Complete documentation within agency guidelines and to meet regulatory requirements.
- Submit completed documentation within one business day after on-call services are provided.
- Communicate regularly with agency nursing staff to review problems or unique issues from on-call hours
- Perform specific assignments as directed
REGISTERED NURSE (RN) REQUIREMENTS:
- NYS licensed and currently registered as a Registered Professional Nurse.
- Thorough knowledge of the scope of practice of professional and paraprofessional healthcare.
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
- Ability to speak effectively before customers/patients or employees of the agency.
- Excellent organizational writing, communication and interpersonal skills.
- Ability to apply common sense understanding to carry out instructions furnished in written, verbal or diagram form.
- Required to participate in on-call rotation.
- Home Care experience is a plus, but not required.
