PRN LVN Home Health / Licensed Vocational Nurse

Overview:
The Licensed Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. You will follow the plan of care, provide treatments, and collaborate with the team to achieve positive patient care outcomes.
Duties & Responsibilities:
- Provide direct patient care as defined in the State's Nurse Practice Act
- Implement the plan of care initiated by the registered nurse
- Accurately and timely document patient care consistent with the plan of care
- Educate and provide information to patients, families, and caregivers relevant to the diagnosis and plan of care
- Coordinate Home Health services and report needs for other disciplines
- Effectively and efficiently use equipment and supplies
- Participate in personal and professional growth and development
- Perform other duties as assigned by the registered nurse

Job Requirements:
- Graduate of an accredited practical nurse or vocational nursing program
- Three years of nursing experience preferred, with a preference for community/Home Health or medical/surgical experience
- Current license as a licensed practical nurse or licensed vocational nurse in the state
- Compliance with accepted professional standards and practice
- Good verbal and written communication, and organizational skills
- Must be a licensed driver with an insured automobile in good working order
- Must possess and maintain current CPR certification
