We are seeking a part-time Peer Support Specialist TCL to provide Peer Support Services to individuals with Mental Health and/or substance use disorders with Permanent Supportive Housing and participating in the Transition to Community Living program. The intent of the service is to increase an individual’s ability to live successfully in the community by maintaining tenancy. Peer Support Services with a Tenancy focus is appropriate for individuals who are currently in stable housing but have continuing needs around maintaining tenancy, community inclusion, building supports, and enhancing quality of life. Qualifications: Highschool Diploma/GED and NC Certification program for Peer Support Specialists.

We are seeking a part-time Peer Support Specialist TCL to provide the following services:
The primary purpose of this position is to provide Peer Support Services to individuals with Mental Health and/or substance use disorders with Permanent Supportive Housing and participating in the Transition to Community Living program. The intent of the service is to increase an individual’s ability to live successfully in the community by maintaining tenancy. Peer Support Services with a Tenancy focus is appropriate for individuals who are currently in stable housing but have continuing needs around maintaining tenancy, community inclusion, building supports, and enhancing quality of life. This service is intended as a step-down from more intensive supports (e.g. Community Support Team, Transition Management Service) and to provide a ‘light touch’ for individuals who have already achieved stable housing (90 days post-lease signing) but require ongoing support for tenancy.
Description of Responsibilities, Requirements, and Duties:
- Collaborates with the client’s therapists to determine the individual’s status and response to treatment; symptom assessment and management;
- Provide individual supportive counseling using the WRAP model;
- Assists and supports activities-of-daily-living, social, interpersonal relationship, and leisure-time-activity services;
- Provide support services or direct assistance to ensure that clients obtain the basic necessities of daily life;
- Educates, supports, and consults with individuals’ families and other major supports.
- Provides expertise that professional training cannot replicate.
- Draws upon personal life experience in his/her work with clients and feels comfortable with the self-disclosure of some aspects of those experiences with mental health issues, substance use issues, and/or treatment and recovery, while maintaining appropriate clinical boundaries.
- Provides highly individualized services in the community and promotes individual self-determination and decision-making.
- Collaborates with therapists and medical providers to promote a culture in which each individual’s point of view and preferences are recognized, understood, respected and integrated into treatment, rehabilitation, and community self-help activities.
- Fostering client’s development of healthy relationships by encouraging participation in community activities and providing services within the community primarily face-to-face.
- Completes treatment notes, goals, and plans for the medical record (e.g., service notes, input into person-centered plan, etc.)
- Works a non-traditional schedule and make themselves available as needed to meet with clients as frequently as needed and at times convenient to them, be available after-hours and go to clients’ homes quickly after hours as needed in crises. Also, available to clients 24/7 in the event of a crisis
