To plan, implement, coordinate, and manage care services to ensure safe and effective patient care.

This is an exciting opportunity to join our cardiovascular service line team and provide care to our Pediatric Heart patients and their families. This Care Coordinator will work with two other clinical Nurse Navigators and an Administrative Coordinator, and deliver increased communication with referring providers, and most importantly, serve as a dedicated concierge service to our patients and families who require subspecialty care. This professional will have the opportunity to work throughout Primary Children’s Hospital, in all areas of cardiovascular care and focus on kids pre, during, and post heart interventions as well as coordination for subspecialty clinics. We are a highly multidisciplinary team giving you the opportunity to collaborate with physicians, nursing staff, allied health professionals, and other APPs within the Heart Center. We are a fast paced unit who takes pride in delivering excellent care as a cohesive team. This position will have a dual reporting relationship to the Service Line Director and Division of Cardiology Administrative Manager. This role supports the Primary Children's Heart Center Nurse Navigation and Outreach team.

Pointed Responsibility: Responsible for the care coordination of patients traveling to Primary Children's for Interventional Cardiology (Cath / Electrophysiology) procedures. Additional responsibilities include Cath / EP program quality oversight and database management (IMPACT Registry and Society of Thoracic Surgeons (STS)).

Role will be hybrid with flexibility and balance with virtual / in-person options.

Job Essentials

Provides for outreach program development and implementation by researching evidence based clinical practice processes, develops strategies and action plans to ensure program goals reflect best practice.

Accountable for care coordination including assessment, individual plan of care, education, patient involvement and choice, resource coordination, ongoing monitoring and care conferencing according to hospital policy.

Assists health care providers in collaborative interdisciplinary teaching for diagnostic specific patients, providing materials and literature where appropriate. Ensures effective planning and arranging for needed services upon discharge. Evaluates the effectiveness of discharge process by monitoring 30-day readmissions.

Completes necessary teaching for identified clinical program.

Evaluates patients for meeting appropriate admission criteria and appropriate level of care.

Identifies and provides care management for patients with complex needs, prolonged length of stay and/or frequent utilization of acute care services. Monitors and evaluates the effectiveness of care management activities in reaching desired patient outcomes on an ongoing basis.

Minimum Qualifications

Current RN Licensure in the State of Utah.

Basic Life Support (BLS) Certification

Three years clinical hospital experience.

Exceptional oral and written communication skills.

- and -

Strong ability for conceptual thinking, problem resolution and planning skills.

Preferred Qualifications

Bachelors degree in Nursing (BSN) from an accredited institution (education will be verified).

Masters degree.

Clinical Project Management experience.

- and -

Data Management experience.

Physical Requirements:

Ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and be able to assess patient needs.

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, etc
