School Based Health Center (SBHC) Patient Support Specialist (PSS)
10 or 12 Month Full-Time positions for Talbot, Caroline, and Queen Annes Counties
Job Summary: The School Based Health Center (SBHC) Patient Support Specialist (PSS) performs a range of clinical, administrative, and staff support to multiple SBHC sites. The PSS ensures that the SBHC operates efficiently and effectively daily, and provides direct support to the SBHC medical, dental, and behavioral health providers. This is a 10 or 12 month full-time position Talbot, Caroline, and Queen Annes counties.
Required Skills/Abilities:
- Valid Driver’s license
- Bilingual language skills (Spanish/English) a plus
- Must be proficient in computer and basic math skills
- Demonstrated experience providing exceptional customer service consistent with AIDET
Education and Experience:
- High School Diploma or equivalent
- Experience working in behavioral health or medical setting, preferred
- Working knowledge of automated patient data systems preferred

Duties/Responsibilities:
- Prepare, distribute, collect, and maintain SBHC/SBDP enrollment forms each school year
- Greet the patient and provide quality customer service in person and on the telephone
- Pre-registration, registration, appointment confirmation, scheduling
- Receive co-payments and provide receipts to patients
- Post charges and payments to the practice management system.
- Verify insurance information and assist with enrollment in sliding fee
- Complete patient referrals
- Case management for behavioral health patients
- Coordination of care between the medical, dental, and behavioral health staff
- Participate in school and community activities to promote the SBHC program
- Communicate with school personnel, Choptank Community Health System Administration and co-workers for schedule changes, closings, etc.
- Maintains inventory and accurately document and update logs according to SBHC policy and procedures
- Prepare chart (obtain vitals as needed), supplies and supporting paperwork for provider
- Regular, reliable attendance is a requirement of this job.
