Position Description
Position Title: Patient Services Representative
Description: The PSR performs duties essential to the patient visit.
Classification: Non Exempt/Full-Time/Hourly
Supervision: Reports to the Support Services Supervisor
Completion of all compliance training and educational programs is a requirement of employment.
Responsibilities:
- Maintains strict patient confidentiality at all times
- Provides high quality customer service
- Schedules patients for testing’s and appointments according to protocol
- Assists with testing/referral follow up
- Confirms appointments and sends pre-visit for all providers on a daily basis
- Receives and directs all questions from patients
- Receives and directs phone calls to appropriate personnel according to protocol
- Prepares daily logs
- Assists patients with the completion of medical information forms
- Performs financial transactions for patients
- Obtains patient signatures for consent according to policy via paper and electronically
- Completes return work/school slips in coordination with provider approval
- Documents phone calls/conversations in record as needed according to protocol
- Performs data entry
- Assists in maintaining medical records according to policy
- Performs copying/scanning/printing medical records
- Assists in maintaining office equipment
- Assists in maintaining the organization and cleanliness of the office
- Assists other team members as needed
- Directs patient complaints to appropriate personnel
- Demonstrates effective communication skills
- Responds to crisis situations according to policy
- Maintains knowledge of EHR, practice management system, scanning program, and patient check-in system along with CHS policies and procedures
- Greets and directs visitors and patients
- Attends all required department and staff meetings
- Requests supplies as needed
- Utilizes supplies in a cost-effective manner
- Participates in organization committees as assigned
- Must be familiar with the operation of all CHS offices and be available to work varying hours as necessary
- Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG Guidelines, other State and Federal laws). This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, and supervisor any suspected unethical, fraudulent, or unlawful acts or practices

Essential Functions:
Physical Factors
Lifting: up to 30 pounds occasionally
Carrying: up to 30 pounds occasionally
Standing: 30-60 minutes at a time throughout the day
Walking: 5-10 minutes at a time throughout the day
Sitting: 15-120 minutes at a time throughout the day
Driving: 10 minutes to 4 hours at a time routinely
Vision: color distinction necessary
Hearing: no greater loss than 40dB average for 500, 1000, 2000 Hz
Qualifications:
- High school diploma or GED required
- Ability to work with a culturally diverse group of people
- Ability to work as a team member
- Ability to read and write in English
- Data entry experience required
- Knowledge of computers required
