The Patient Navigator ensures optimized interaction between a company and members. The Patient Navigator assumes ownership and leads advanced and highly specialized administrative/operational/customer support duties that require independent initiative and judgment. Will aid in the development of a Medicare Advantage training program for center staff and providers, responsible for patient and broker engagement.
CenterWell is seeking a dedicated, compassionate, and experienced - Patient Navigator- who is interested in growing with the company and being a part of something great!
Location: CenterWell Senior Primary Care office located throughout metropolitan Las Vegas, NV
Patient Navigator Role Overview:
  • Ensures optimized interaction between a company and members
  • Assumes ownership and leads advanced and highly specialized administrative/operational/customer support duties that require independent initiative and judgment
  • Aids in the development of a Medicare Advantage training program for center staff and providers, responsible for patient and broker engagement
  • Works independently and have excellent time management skills
  • Collaborator of relationships between the physician/staff and patients to help educate patients on Medicare FFS vs Medicare Advantage
  • Educate staff on OEP, SEP and AEP
  • Develops content for Medicare Advantage training program for center administrative staff, clinical staff, and physicians.
  • Hosts quarterly education meetings within each Centerwell location alongside brokers to educate staff on benefit details, answer questions and address issues in regards to Medicare MA or FFS plans.
  • Launches webinar series for clinical cohorts
  • Collaborates with brokers to conduct Medicare 101 classes for administrative staff and consumers
  • Develops patient concierge program role by implementing patient outreach to follow up on visits and educate patients about Medicare
  • Conducts visit follow-ups with original Medicare patients; customer service focus that will conclude with conversation around Medicare Advantage
  • Begins clinical team engagement
  • Assists front desk staff in difficult conversations on Medicare education
  • Position will be a patient facing position in clinic, as well as participating in community events

Use your skills to make an impact
Required Qualifications
  • 2 - 4 years of consumer operations health care experience
  • Extensive Medicare knowledge and compliance points.
  • Excellent customer service skills both telephonically and in person.
  • Proficiency in Microsoft Office Programs including Word, PowerPoint, Excel and Access
  • Previous experience with coordinating and leading projects and tasks

Preferred Qualifications
  • Associate's Degree
  • 5+ years Medicare/Insurance experience

Patient Navigator Working hours:
Scheduled 40 hours per week
Monday to Friday 8AM-5PM
Local travel may be required; Mileage is reimbursed

Language Proficiency Testing:
Bilingual English/Spanish. Must be able to speak, read and write in both languages without limitations or assistance. See Additional Information on testing. Any Humana associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay decisions will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$47,000 - $64,700 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
