Patient Navigator Human Services, HIV and Aging

40 hours per week, day shift

Please note:this is a grant-supported (time-limited) position based in the outpatient clinic with some duties requiring work outside normal business hours. Some duties may be performed by telework with supervisor approval and completion of a telework agreement.

JOB SUMMARY

Full-time, grant-funded (time-limited) position supporting the efforts of the Atrium Health Wake Forest Baptist ID Specialty Clinic (IDSC) Ryan White HIV Program by increasing engagement and retention in care of persons with HIV age 50 and older followed by the program.  Navigator will serve as a liaison connecting older persons living with HIV to aging-related medical care and community organizations and resources.  The navigator will also serve as a resource for clinic staff on issues related to aging with HIV.

EDUCATION/EXPERIENCE

  • Experience with the provision of care coordination services to patients living with HIV and/or from underserved populations with barriers to accessing and maintaining medical care 
  • Bachelor's degree in social work, sociology, public health, psychology, or equivalent combination of education and experience

ESSENTIAL FUNCTIONS

  • Responsible for increasing engagement and retention in care of persons with HIV age 50 and older followed by the IDSC 
  • Serve as a liaison for these patients to access HIV and Aging-related medical care and specialty services and community resources
  • Provide counseling on healthy behaviors and support clients in reducing health-related risk behaviors to promote healthy aging
  • Serve as a resource for clinic staff on issues related to aging with HIV
  • Support a community advisory board focusing on the needs and concerns of patient and community stakeholders regarding aging with HIV
  • Collaborate with teammates in clinical research engaged in research regarding HIV and Aging
  • Connect patients to resources within the Medical Center and larger community to meet the patients' specific needs and address barriers to optimal health such as difficulties with medications, social stigma, family issues, transportation, etc.
  • Function as a resource for patients on matters of financial issues and obtaining access to insurance coverage 
  • Maintain close communications and participate in case review with medical providers, social workers, community-based case managers, and other community-based care providers regarding the plan of care, patient progress, and appropriate interventions 
  • Participate in the Quality Management Program, including membership on the interdisciplinary Quality Committee
  • Attend local, regional, and national meetings and trainings as needed
  • Other duties as assigned

SKILLS AND QUALIFICATIONS

  • Excellent interpersonal skills and ability to interact with Medical Center personnel in various departments as well as outside care providers, community agencies, health departments, etc.
  • Strong communication skills including ability to lead group discussions, deliver information in a manner accessible to a range of clients and care providers, and compose written summaries of activities
  • Capacity to learn electronic medical record 
  • Knowledge of HIV/AIDS disease (HIV modes of exposure and transmission, diagnosis, disease progression, role of HIV viral load and CD4 in monitoring, and familiarity with potential long-term challenges of aging with HIV)
  • Knowledge of the importance of antiretroviral medication adherence to prevent HIV disease progression and reduce risk of antiretroviral drug resistance
  • Personal experience living with HIV preferred
  • Familiarity with dealing with cultural issues in diverse, underserved population 
  • Bilingual in Spanish (preferred, not required)
