We are looking for a Patient Navigator to join our growing team in Akron, OH. This will be a full time, full-benefits, 40 hours a week position. The hours will be Monday-Friday, 8:00am -5:00pm.
The Patient Navigator will be responsible for driving patient and clinical team communication, streamlining coordination of care, and delivering the highest levels of service to our patients. The Patient Navigator will be the essential link between patients, physicians, nurses, and administrative teams to provide the timely communication and support to our patients. This will be accomplished through coordination, by the Patient Navigator, throughout the continuum of care from first contact through discharge.
Responsibilities:
- Serve as critical resource for patients across the patient care team to facilitate ease of and satisfaction with the patient experience and treatment cycle
- Provide first-line contact to support patients as they pursue treatment by helping to manage day-to-day logistics, simplifying clinical services and supporting the education and emotional needs across the continuum of care
- Serve as single and centralized point of communication (in person, by phone, email and text messaging) between patient, healthcare providers, and all admin team members to enhance communication, promote good working relationships, accomplish assigned duties, and improve the delivery of care
- Anticipate the needs of the patient and RGI providers and deliver coaching, communication, and customer service as needed to multi-disciplinary care team
- Track all patient contacts, follow ups, barriers to care and outcomes
- Utilize nAble EMR system to ensure patient records are up-to-date and secure
- Provide support to the team with other administrative tasks as necessary
Requirements
- 2+ years of years of demonstrated service excellence experience; prior progressively responsible experience coordinating patient care
- OB/Gyn or reproductive health care experience desirable
- Strategic mindset and ability to consider implications & impacts to patient experience
- Able to quickly build strong relationships at all levels across the organization
- Strong verbal and written communication skills
- Detail-oriented with strong organizational skills including proactive planning, follow-through and follow-up
- Ability to manage multiple patients in a fast-paced changing environment
- Ability to communicate empathetically and effectively and consistently demonstrate strong emotional intelligence and self-awareness.
- To support patient needs, this role will require flexibility and ability to accommodate an irregular work schedule, when needed.
- Experience in or knowledge of scheduling, clinical terminology, insurance, clinic protocols
- EMR experience required
