The position function as a liaison between assigned patients and the clinical care team, as well as supporting departments, to assure patient's expectations are completely satisfied. Considers and incorporates special needs, such as logistics and scheduling of non-traditional patients. Builds relationships with patients and physicians, to assist in the communication and implementation of a plan of care including appointments, transportation, education, provision and/or enablement of support services and representation within the multidisciplinary care environment. Uses independent judgment, critical thinking, problem-solving and advanced communication skills to achieve the highest level of patient satisfaction in accordance with Baptist health Service standards and in compliance with regulatory guidelines. Must work in collaboration with Nurse Navigator and clinical care staff on resolution of critical aspects of care that require clinical judgement and safe delivery of patient care. Estimated pay range for this position is $18.59 - $22.49 / hour depending on experience.
High School Diploma, Certificate of Attendance, Certificate of Completion, GED or experience required. For Oncology: Oncology Patient Navigator - Certified Generalist (OPN-CG) desired within 2 years of job entry date. For Women's Institute: Certified Navigator-Breast Advocate through the NCBC (National Consortium of Breast Centers) desired within 2 years of job entry date. Associate's preferred or in progress. Required 3 years of relevant work experience in customer service. Healthcare experience desired. Navigation/care coordination experience preferred. Proficient English communication skills required, written and verbal. Bilingual English/Spanish preferred. Proficient computer skills (i.e.: Microsoft Office Word, PowerPoint, Excel, etc.). Minimum Required Experience: 3 years
