Sub-products:
- Manage HTP Fax
- Respond to screening, social media and clinic inquiries (within 24 hrs)
- Manage requests for internal referrals from the internal referral form (same week)
- Input information into and manage referral log
- Analyze Fusion schedule to determine the best recurring schedule plan to align with other services for current patients
- Manage intake paperwork prior to initial evaluation
- Manage Fusion account creation and information input for new clients
- Communicate with caregivers to schedule initial evaluation (in-person, phone and/or email)
- Confirm initial eval and recurring appointment day/time
- Communication with caregivers through in-person, phone and/or email
- Fill advanced schedules of therapists with evals
- Verify insurance for current clients who are internally referred for another discipline
- Hit evaluation target numbers
- Request updated orders on expired referrals (within 100 day window)
- Send discharge information to Referring Physicians upon request
- Log client and insurance information from referral log to Billing Log to ensure billing is aware to notify commercial plan clients of payment requirements
- Send out all evaluation reports to referring physician’s with confirmation of checked in, no show,, canceled or rescheduled their eval within one week of eval
As needed
- Communication with internal and external professionals
- Connect with PR/PCC when necessary
- Inform providers on diagnosis codes to use for internal referrals
- Request new orders with additional diagnosis codes for therapists
- Feeding Therapy: Obtain feeding referrals, schedule assessments and add as 2nd appointments to the recurring schedule
- Share patient appointment records as requested by providers
- Manage the Callback Program as directed
- Follow up on discharged patients 6 weeks following discharge with a goal of 10 calls per week (calls are completed if there is a need to fill for certain services ie. OT only) follow the discharge paperwork log and document calls in patients’ case notes in Fusion
