• Strong communication, interpersonal, and problem-solving skills, with the ability to thrive under pressure.

• Familiarity with identifying and addressing challenges related to patient care, healthcare access, and insurance.

• Understanding of relevant healthcare laws and regulations, including HIPAA.

Responsibilities:

As a Patient Care Coordinator, it is imperative to possess the ability to identify and suggest patients who may be encountering prevalent mental health conditions. Your role assumes critical significance in patient care through the facilitation of essential procedures, including but not limited to, form completion, coordination of telehealth consultations, and fostering transparent communication with healthcare professionals. Your focus on Mental Health and Nutritional therapy services will contribute directly to improving healthcare delivery and patient satisfaction within our organization.

• Provide guidance and support to patients in understanding mental health and nutritional treatment plans.

• Foster communication and collaboration among healthcare providers and medical professionals.

• Advocate for patient adherence to prescribed treatment plans and nutritional choices.

• Address barriers to treatment plan compliance with empathy and sensitivity.

• Ensure accurate and secure maintenance of Patient Health records.

• Work under HIPAA regulations.

• Provide immediate support during mental health crises and collaborate with intervention teams as needed.

• Conduct regular health assessments and outreach activities to promote mental and nutritional health awareness in the community.
