What You’ll Do

  • As a Dialysis Registered Nurse you will be able to utilize your natural gifts as a patient advocate to drive differential results for dialysis patients in Skilled Nursing Facilities.
  • We have partnered with local Skilled Nursing Facilities providing on-site Home Hemodialysis to Skilled Nursing Facility patients.

Our current hours of operation are Monday - Saturday / 7am - 5pm (hours may vary based on patient census and care required).

Minimum Qualifications

Education:

  • Graduation from an accredited Registered Nurse, RN Program.
  • Current state license as a Registered Nurse, RN
  • Current American Heart Association BLS - CPR certification.

Required:

  • 12 months of Hemodialysis experience

Key Responsibilities

  • In conjunction with the Regional Director, ensures dialysis unit operates in compliance with all ESRD, HCFA, federal, AAMI, OSHA and company policies and procedures.
  • Responsible for training and development of Skilled Nursing Facility staff while providing clinical oversight to non-licensed support team delegating tasks as appropriate
  • Knows and follows policies and procedures under CMS ESRD and state regulations and CDI Health; monitors staff compliance of same
  • Monitors and assures patients receive adequate medical attention during dialysis treatments and pertaining to dialysis care while a patient of the Skilled Nursing Facility in the dialysis unit.
  • Conduct comprehensive assessments that include the medical, behavioral, pharmaceutical, and social needs of the patient, identify gaps in care and barriers to good health;
  • Based on this assessment, and in conjunction with the patient care team, create and implement a care plan that will address identified needs, remove barriers to care, and improve the health of the patient
  • Coordinate care by serving as the advocate and resource for the patient, their family, and their provider(s);
  • Facilitate continuity of care, spanning settings such as the home, hospital, skilled nursing facility, and acute care facility;
  • Manage patients during periods of transitions of care minimize avoidable re-admissions;
  • Assess the patient's knowledge of their renal condition and provide education and self-management support;
  • Provide ongoing reassessment and follow-up to improve patient outcomes
  • Monitors quality control measures to assure continuous quality of patient care.
  • Maintains open communication with Skilled Nursing Facility Administrator, Nephrologist and CDI Health management and staff.
  • Utilizes resources to improve skills, knowledge and practice in order to assure continuous quality of patient care.
