Oak Leyden has an exciting new opportunity for a Residential QIDP.
Do you have a bachelor degree in Social Services or related field and at least 1 year of experience working with Intellectually and Developmentally disabled individuals?

The QIDP is responsible for the overall programing for participants to whom she/he is assigned (2 CILA Group Homes/ 12 Participant caseload). This includes, but is not limited to developing the Implementation Strategy portion of the Person Centered Plan, ongoing coordination of services for participants, ensuring financial & medical benefits remain intact, acting as the main spokesperson between Oak Leyden and guardians / family members and facilitating the resolution of issues/problems involving participants by holding Special Staffing’s. The Q will assure full compliance with and adherence to all applicable Federal, State and Agency rules, regulations, policies and procedures. Responsibilities include but not limited to the following:
  • Participation in the Person Centered Planning process.
  • Provide, coordinate and monitor the ongoing need for such services as counseling, Supported Employment, BCBA services, banking and financial support, as well as work with Home Managers to ensure participant’s personal needs are being met.
  • Ensures that financial entitlements - Social Security (SSI and SSDI) are maintained, that eligibility for Medicaid and SNAP remains in place and that participants have access to their finances. Assist in the maintenance of participant fund accounting as directed.
  • Acts as the main spokesperson between Oak Leyden and guardians / family members to communicate program information, address problems and concerns and share successes.
  • Participates in the intake and discharge process for participants assigned to them.
  • This position requires being on call outside of regular shift hours for participant issues and emergencies. Guardians must be informed immediately of any emergencies that occur so that may require an evening phone call/discussion.
Qualifications:
  • Current QIDP status.
  • This position requires a Bachelor of Arts or Science degree in a Social Service discipline as required for QIDP qualification. The minimum required applied, professional experience is one (1) year, the preferred experience is not less than three (3) years, one of which including supervisory experience.
  • Must be highly skilled in communication, have a clear and working knowledge of application rules and regulations from the Illinois Department of Human Services, and the policies and procedures of Oak Leyden.
  • Ability to pass background, physical, drug, TB & lift test (Must be able to lift over 50 pounds), have a clear Illinois Driver’s License and be willing to drive an agency vehicle(s) and own and use a personal vehicle while engaging in business activities.
  • The QIDP must keep current in annual CEUs and attend internal and external training to meet agency and regulatory standards.
