Custom Home Care (CHC) is looking for a part-time Wellness Nurse. As the Nurse, you will play an integral role in the agency’s wellness offices located in premium independent senior living communities.
Salary up to $40/hr *depending on certification (LPN/RN)
Wellness Nurse Duties:
- Access “new clients” to development a plan of care to determine what type of care is needed (hourly/live in) and reassess current clients care
- Instruct and advise current clients and/or families on their care
- As well as advise CHC’s Caregivers of those care plans
- Address/manage caregivers’ concerns and make adjustments to the clients in-home Care Plan and follow up with client’s family and/or doctors if necessary
- Keep the Community management aware of resident change of health
- Perform client visits and monitor their conditions
- Build and maintain strong relationships with referral sources to expand our client base
- Proactively seek out and engage with potential referrals and clients in need of home care services
- Strong working knowledge of technology (i.e. Microsoft Office, WellSky, etc)
