***Part-time position working approximately 24 hours per week. This 24-hour schedule will fall between Monday - Friday, 7:00 AM - 3:30 PM. The specific scheduled hours will be determined between the right candidate and hiring manager.

Position Objective:

TheCES Assessment Specialistposition is responsible for enrolling new individuals and/or families experiencing homelessness (neighbors) into the Indianapolis Continuum of Care's Coordinated Entry System (CES). The assessment is used to determine eligibility and prioritization for rental assistance and housing support services. Assessments may take place within the Horizon House service center or onsite at community partner organizations. This position functions in collaboration with the service center's case management and guest services teams as well as community partners to link neighbors experiencing homelessness to resources, services and supports for long-term housing stability. This is a part-time, 24 hour per week, position. This 24-hour schedule will fall between Monday - Friday, 7:00 AM - 3:30 PM. Requirements:

Training/Education:

  • Bachelor's degree preferred; equivalent experience required
  • Fluency in Spanish is preferred

Experience/Skills:

  • Prefer one to two years' experience in a program providing services to individuals/families experiencing homelessness or poverty (may include educational/academic, internship and/or volunteer experience)
  • Requires the ability to support individuals experiencing homelessness by developing professional helping relationships with neighbors, agencies, and community partners
  • Requires strong written and verbal communication skills;
  • Requires knowledge of human behavior and skills to assess client strengths and needs;
  • Prefer working knowledge of community resources;
  • Requires strong organizational skills with attention to detail;
  • Proficient in all Microsoft applications and ability to become proficient in required database tracking systems

Essential Functions:

Assessment and Triage:

  • Work with a diverse clientele to conduct thorough assessments using format and tools determined by Indianapolis Continuum of Care.
  • Enroll neighbors into the CES process and provide appropriate, timely interventions and referrals to address immediate needs.
  • Assist in managing crises and emergencies assigned as needed.

Care Coordination:

  • Coordinate with co-located partners and other community organizations, to secure and manage all documentation required for the CES assessment.
  • Maintain documentation and client contact necessary for CES assessments to remain active, per guidelines established by the Continuum of Care.
  • Participate in weekly Case Conferencing meetings to represent and advocate for neighbors in the housing referral process.

Record Keeping:

  • Maintain electronic and paper documentation of client records in compliance with agency and external policies, procedures, and any applicable licensing standards.
  • Manage enrollment, during enrollment and exit documentation per CES guidelines.

Legal:

  • Observe protocol for confidentiality and ethical conduct/behavior, including but not limited to maintaining appropriate professional boundaries.
  • Take appropriate action according to established protocol of the organization and/or local/state ordinances/laws.

Education/Training:

  • Attend required agency and/or community education and training activities, including all sessions required to secure and maintain position as CES Assessor.
  • Assist in training new staff, community partners and interns as appropriate.
  • Offer peer assistance to colleagues as appropriate.
  • Contribute regularly to agency and team meetings; represent the agency at community meetings as requested.

Hours:

  • Part time position; 24 hours per week- typically Monday through Friday with occasional evening or weekend hours, as-needed.

General Assistance:

  • Assist with other duties as assigned by the Clinical Supervisor, Program Manager or other agency leadership.

Contacts and Liaisons:

The CES Assessment Specialist will regularly communicate with Horizon House employees, outside vendors and other individuals as deemed necessary in order to fulfill the responsibilities of the position.
