Home Health Registered Nurse (RN) - Highly Skilled and Compassionate

Job Summary:

The Registered Nurse plans, organizes, and directs home care services with a focus on community health education and experience. As a professional nurse, you will utilize the resources of the community to develop and implement individualized care plans for patients and their families.

Duties & Responsibilities:

Patient Care:

• Develop and initiate the plan of care in partnership with the patient, representative, and caregiver.
• Provide preventative and rehabilitative nursing procedures as ordered by the physician.
• Offer patient, caregiver, and family counseling and education.
• Prepare clinical notes and communicate with the physician and other healthcare practitioners.

Communication:

• Update the primary physician regularly and communicate changes in the patient's condition.
• Coordinate care plans with community health-related persons.

Additional Duties:

• Participate in quality assessment and performance improvement programs.
• Participate in on-call duties as defined by the policy.
• Ensure necessary equipment and services are available.
• Instruct, supervise, and evaluate Home Health aide care.
• Supervise Licensed Practical Nurses (LPNs)/Licensed Vocational Nurses (LVNs) and/or Home Health Aides (HHAs)/Certified Nursing Assistants (CNAs) as directed.

Job Requirements:

• Graduated from an approved school of nursing and licensed in the state where practicing.
• Minimum of two years of experience, with at least one year in public health or home care nursing preferred.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Self-directed and able to work with minimal supervision.
• Excellent observation, problem-solving, and communication skills.
• Ability to organize and prioritize workload independently.
• Able to lift, position, or transfer patients and supplies.

We are looking for a Part Time Weekend On-Call RN. Schedule is Friday, Saturday, Sunday for scheduled and on-call visits.
