Salary - $48,000 to $50,000

Full COVID-19 vaccination required

The Parents as Teachers Parent Educator (PAT PE) is responsible for initiating and maintaining regular, long-term contact with families through home visits. During these visits PAT PEs concentrate on family-centered, strength-based, goal-oriented services to assist families with issues involving the parent-child relationship, improving the family support system, family problem solving, and advocating for the client. Referral and monitoring of health care usage and child development are also stressed.

Essential Functions:

  • Travels door-to-door or site-to-site to locate and communicate with pregnant women, directs them to needed services, and encourages them to obtain prenatal care
  • Contacts women by telephone, mail or home visits to assure proper follow through in making prenatal visits and in obtaining other services
  • Supports clients to plan for their own prenatal services, social services, housing needs, and job opportunities
  • Educates clients about parenting and prenatal care related topics
  • Schedules prenatal, postpartum and well community resources needed by clients
  • Maintains relationship with identified clients and provides emotional and/or maternal support as needed
  • Refers families needing more intensive follow-up to appropriate agencies
  • Keeps careful records of all contact made and follow-up as necessary. Keep accurate records of services received by clients
  • Ensures confidentiality of client records, sharing pertinent information only with the client's approval
  • Meets with Supervisor regularly to report and discuss work
  • Attends initial and ongoing training sessions on maternal and child health, welfare, housing nutrition, substance abuse, counseling, parenting, infant and family support issues
  • Attends community meetings, health fairs and represents SNJPC as requested by Supervisor
  • Performs other duties as assigned


Requirements

  • Associate Degree or participation in courses towards an Associate Degree or completion of a Community Healthcare Worker Certification course
  • Knowledgeable about community resources
  • Ability to work well with a variety of people and develop rapport with clients from varied cultural/ethnic backgrounds
  • Desire to expand knowledge base
  • Committed to better health care in community
  • Effective oral communication skills; Bilingual strongly preferred; ability to keep written records
  • Valid driver's license, car and current insurance policy

Physical Requirements:
  • Must be able to see to read documents and hear well enough to communicate with others
  • Must be able to write, type on a computer, and use a telephone
  • Must be able to operate a motor vehicle
  • Must be able to lift up to 10 pounds
  • Additional requirements to be determined with input from Director and HR

Work Environment:

Travels regularly to sites where traffic, weather, and other environmental conditions may be encountered; otherwise works in a climate-controlled professional office environment generally free from hazards and other adverse working conditions. Works a flexible schedule that includes some weekend and evening hours
