Description of Primary Responsibilities
  1. Provides overall management and direction to departmental/division staff.
    1. Responsible for performance management of departmental employees, including all managers, supervisors, and employees in the department, including recognition, performance evaluations, formal coaching and counseling, and making decisions or recommendations regarding necessary disciplinary actions.
    2. Responsible for recommending hiring or firing, and the advancement and promotion of managers, supervisors, and employees in the department, or any other change status of manager, supervisors, and employees in the department employees.
    3. Demonstrates core leadership behaviors and team one approach.
    4. Demonstrates a high level of emotional intelligence.
    5. Creates a culture of accountability and excellence.
    6. Drives execution and innovation.
    7. Ensures division alignment with organizational culture and strategic vision.
    8. Ensures the development of and successful execution of an action plan across the assigned employee base to support the strategic direction of the organization and obtainment of operational goals for assigned departments/division, effectively leading change when necessary.

    10. Ensures team members of all assigned areas of responsibility are fully functional and performing at a world class level.
    11. Ensures development of department/division managers and supervisors receive instruction/training that is in compliance with training plan, including on the job training to develop department employees. Works with manager and/or supervisor to ensure necessary remediation is taken with department/division employees assigned.
    12. Assists with the development of assigned departmental/division budget(s) and monitors budget to ensure expenses do not exceed budget.
    13. Ensures regulatory compliance for assigned departments/divisions, and compliance with all workflows, policies, and procedures.
    14. Recommends workflow, policy, and procedure changes based on observations from performance metrics, outcomes, and feedback from assigned leadership team members.
    15. Ensures department maintains compliance with all employee related reporting and tracking.
  2. Responsible for delivery of care and clinical outcomes.
  3. Provides medical guidance and supervision of medical services activities.
  4. Provides leadership and medical expertise in the development of medical policies, procedures, and guidelines.
  5. Responsible for the development of FHCN PACE clinical standards and medical practice guidelines and protocols.
  6. Provides oversight of the primary care staff to include physicians and midlevels.
  7. Functions as a primary care physician within the interdisciplinary team (IDT), including providing clinical supervision to medical care personnel such as midlevels.
    1. Performs comprehensive history and physicals on new referrals.
    2. Performs interval history and physicals on established participants, completed quarterly or as indicated.
    3. Participates as needed in the delivery of primary care.
    4. Coordinates the 24-hour care delivery.
    5. Documents participant changes appropriately in the medical records and communicates participant changes to IDT in a timely manner.
    6. Participates as a member of the IDT. Attends staff members and takes part in participant care planning.
  8. Responsible for the compliance and oversight of the Medicare Part D Drug Management Program.
  9. Provides oversight of the Quality Improvement (QI) Plan.
  10. Reviews all quality-of-care issues and oversees the development and implementation of quality-of-care corrective action plans.
  11. Participates in the oversight, training, and education of the interdisciplinary team.
  12. Coordinates performance appraisal of the staffing under his/her oversight, including providers.
  13. Develops educational and other programs to build the skills of participating providers.
  14. In conjunction with the PACE Program Director, manages all communication with the provider network.
  15. Represents FHCN PACE to external agencies, professional groups, and regulatory agencies and organizations as required.
  16. Demonstrates necessary skills and knowledge as outlined in position-specific competency requirements.
  17. Meets Network and credentialing expectations for licensure and employment.
  18. Performs other duties as assigned.

Description of Primary Attributes
Professional & Technical Knowledge:
  1. Possesses specific advanced knowledge and skills, including written and verbal communication skills, computational, computer and technical skills, and mathematical knowledge frequently acquired through completion of a Bachelor's Degree or Master's Degree program with a recognized major or comparable experience, and; Board-certified in Internal Medicine or Family Practice with advanced certification in geriatrics preferred.
  2. Must have experience working in a managed care environment and working with peers and other health providers to resolve utilization, quality management, performance improvement, pharmacy and therapeutics, peer review, credentialing, and physician leadership issues.
  3. Minimum three years of experience in a lead administrative role or equivalent.
  4. Possesses the ability to act within the scope of his or her authority to practice medicine, meet a standardized set of competencies for the specific position description established by FHCN PACE and approved by CMS before working independently.
  5. Performance of the duties and responsibilities of the job requires extensive formal training in medical studies, including an understanding of the application of the theory and practices of medicine.
  6. Either have one year of experience working with a frail or elderly population or, in the absence of such experience, to receive appropriate training from FHCN PACE on working with a frail or elderly population upon hiring.
  7. Able to work within an interdisciplinary team setting.

Technical Skills:
  1. Ability to prepare more complex documents in Microsoft Word, including creating tables, charts, graphs and other elements.
  2. Ability to use Microsoft Excel to analyze data, including the use of formulas, functions, lookup tables and other standard spreadsheet elements.
  3. Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions and other elements.

Licenses & Certifications:
  1. Licensed in the state of California as an MD or DO.
  2. DEA registration and the ability to obtain and maintain staff privileges at FHCN PACE contracted agencies.
  3. Medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Communications Skills:
  1. Job duties require the employee to effectively communicate, verbally and in writing, their opinions and extrapolations of information they collect and synthesize/analyze.
  2. Responsible for the resolution of conflicts that may arise because of disagreements between employees, between employees and customers/clients, or with the public, other legal entities or governmental authorities.
  3. Compiles, analyzes and prepares information in an effective written form, including correspondence, reports, articles or other documentation.
  4. Effectively conveys technical information to non-technical audiences.

Pay Scale:
Min Salary Rate: $271,463.77
Max Salary Rate: $434,342.03
