Transition's Health Partners was established in 2013 as a physician-owned local Colorado company comprised of Transitions Home Health Care, Transitions Chronic Care, and now Transitions Wellness & Therapy Care. Our unique approach promotes well-coordinated care as patients move across care settings and are discharged to home. This strengthens the role of the primary care provider and specialists while producing better health outcomes for our patients.

Physical Therapy Job Summary:

Evaluates, plans and administers comprehensive physical therapy treatment/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative occupational therapy as prescribed by a physician.

Clinical setting is outpatient rehabilitation services for residents in their place of residence.

We have PRN, part-time, and full-time positions available! We are looking for a strong clinician who will help foster our relationship with the community!

Level of Responsibility:

Works under the supervision of the Clinical Manager or their designee. Supervises physical therapy assistants as assigned.

Duties and Responsibilities:

  1. Ongoing interdisciplinary assessment of the patient.
  2. Develops, initiates and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver (s).
  3. Provides skilled therapy services per physician orders, the patient’s plan of care, clinic policies and professional standards.
  4. Performs all aspects of patient/client care in an environment that optimizes safety.
  5. Ensures that changes in patients’ clinical status are communicated to other healthcare team members and clinic office staff in a timely manner.
  6. Obtains all necessary physician orders and collaborates with the physician as the patient's needs dictate. Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
  7. Coordinates patient care and participates in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
  8. Educates and counsels patients, their families/caregivers and other staff as appropriate, in appropriate physical therapy modalities.
  9. Provides, modifies or discontinues physical therapy modalities based upon an ongoing assessment/reassessment of the patient’s clinical status and in accordance with physician orders.
  10. Assesses the patient for the need for durable medical equipment as well as other adaptive equipment needs.
  11. Develops appropriate home exercise programs tailored to the patient’s clinical status, needs and age.
  12. Provides supervision and mentorship to PT assistants.
  13. Documents all care and services provided according to professional standards and clinic policy and procedures.
  14. Adheres to all clinic policies and procedures.
  15. Completes all mandatory clinic training and in-service activities.
  16. Participates in the clinic’s quality management as appropriate.
  17. Strong documentation skills with experience using objective measures and assessments.
  18. Establishes and maintains effective channels of communication and working relationships.
  19. Supports and maintains a culture of safety and quality.
  20. Maintains confidentiality in all aspects of job performance.
  21. Performs related duties as required.

Minimum Qualifications:

  1. Licensed as a Physical Therapist in the State of Colorado.
  2. Graduate of a Physical Therapist curriculum approved by the Commission on Accreditation in Physical Therapy, or if educated outside the United States, graduated from an education program determined to be substantially equivalent to Physical Therapist Assistant entry-level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association.
  3. Two years’ experience as a Physical Therapist and previous experience working with the geriatric population preferred.
  4. Current Medicare Credentialing a plus!

Knowledge, Skills, and Abilities:

  1. Knowledge of physical therapy practice, with emphasis on the provision of physical therapy services in the home environment.
  2. Ability to foster constructive and therapeutic relationships with patients/clients and their families.
  3. Ability to communicate effectively, both orally and in writing.

Special Requirements:

  1. Current, valid license to practice as a Physical Therapist in the State of Colorado.
  2. CPR certification.
  3. Current, valid driver's license.
  4. Access to a reliable automobile.

Exposure/Risk Category:

Tasks may involve occupational exposure to blood, body fluids or tissue.
