Job description

Care of Patients:


Interprets the appropriateness of a medication order before preparing or permitting the distribution of the first dose

  • Demonstrates ability to use reference sources (books, journals, etc.) to assess appropriateness of drug therapy, particularly in consideration of age, size and renal function of patient.
  • Ensures all medication orders are delivered in an accurate and timely manner.
  • Recognizes non-formulary drugs, interacting drugs or therapy intervention targeted drugs, and offers information on alternate choices to physicians, consistent with hospital formulary system.
  • Ensures adequate supply of medications and supplies are available beyond the immediate shift. Uses drug supply channels, usual, or alternate sources, if necessary, to obtain needed medications.


Departmental Process, Standards and Performance Improvement


Participate in department programs as related to medication use

  • Demonstrates proactive approach to ensure high quality of drug therapy.
  • Consistently completes daily audits of narcotic sheets or ADM records and maintains perpetual inventory of narcotics per policy and procedure.
  • Appropriately and consistently carries out the therapeutic substitution Program.
  • Investigates Medication Occurrence Reports for opportunities to reduce events.
  • Recognizes specific area of knowledge deficiencies and demonstrates ability to improve and increase knowledge base and practice skills.
  • Accepts and completes special assigned or delegated tasks on projects which impact departmental or hospital operations or improve patient care.
  • Verify all dosages and regimens with a published source for each order.
  • Double check all dosage calculations based on patient-specific parameters provided on the patient care plan.

Dispense medications according to laws and policies and procedures

  • Consistently demonstrates appropriate knowledge and skills for identified population in relationships to dispensing of medication
  • Medications contain all necessary and/or appropriate ancillary labels/information
  • Adheres to appropriate safety and quality assurance practices during Medication preparation and documents competency with supervisor at least every 12 months according to USP standards.
  • Medications are processed and dispensed for timely delivery
  • Monitors accuracy of filled orders and initials label before delivery
  • Monitors that all medications are unit dosed with the lot number, expiration date, and brand on the package
  • Ensures charges are appropriately completed/ reconciled within 24 hours; reports exceptions to DOP within 24 hours.

Education:
Bachelor’s degree in Pharmacy or Pharm.D. degree.
Additional Certification & Licensures:
Active TN license as a pharmacist.
