Prime Home Health is seeking a highly skilled and compassionate Occupational Therapist to join our team. As an Occupational Therapist in the Home Health setting, you will play a critical role in helping individuals achieve independence and improve their quality of life in the comfort of their own El Paso homes. Your primary responsibility will be to assess clients' functional abilities, develop personalized treatment plans, and provide therapeutic interventions to address their specific needs. Responsibilities:
- Conduct comprehensive evaluations of patients' physical, cognitive, and psychosocial functioning to determine their occupational therapy needs in a Home Health setting.
- Collaborate with clients, their families, and other healthcare professionals to establish realistic and measurable goals for therapy interventions.
- Design and implement individualized treatment plans based on the evaluation findings and in accordance with professional standards and regulatory requirements.
- Provide therapeutic interventions and instructions to improve clients' independence in activities of daily living, including but not limited to self-care, mobility, home management, and community integration.
- Utilize a variety of evidence-based techniques, adaptive equipment, and assistive devices to maximize clients' functional abilities and promote their overall well-being.
- Monitor and document clients' progress, adjustments made to treatment plans, and any changes in their condition or functional abilities.
- Educate clients, families, and caregivers on therapeutic techniques, safety measures, and strategies to enhance clients' independence and prevent future injuries.
- Collaborate with other healthcare professionals, such as physicians, nurses, and social workers, to ensure holistic care and effective coordination of services.
- Maintain accurate and timely documentation of evaluations, treatment plans, progress notes, and discharge summaries as per regulatory and organizational requirements.
- Stay updated with advancements in occupational therapy practice, research, and technology through continuous learning and professional development activities.
- Adhere to ethical and professional standards of practice, maintaining confidentiality and promoting a positive and respectful environment for clients and colleagues.
Qualifications:
- Bachelor's or Master's degree in Occupational Therapy from an accredited program.
- Texas state license as an Occupational Therapist.
- Proficiency in using Kinnser EHR, Slack and Bamboo (or other systems) for documentation and communication purposes.
- Valid CPR BLS certification
- Proven experience in providing occupational therapy services, preferably in a Home Health or community-based setting.
- Strong knowledge of human anatomy, physiology, and occupational therapy principles and techniques.
- Ability to perform comprehensive assessments, develop treatment plans, and deliver evidence-based interventions based on individual clients' needs.
- Excellent interpersonal and communication skills to effectively interact with clients, families, and interdisciplinary team members.
- Demonstrated ability to work independently, manage time efficiently, and prioritize workload effectively.
- Empathy, compassion, and a genuine desire to improve the lives of individuals receiving therapy services.
- Valid driver's license and reliable transportation to travel to clients' homes primarily within the northern San Antonio area.
