Description:
POSITION PURPOSE
Depending on the Health Ministry, a pharmacist may provide pharmaceutical care for neonatal through geriatric populations. Care is provided according to professional standards and statutory requirements. Directs the functional activities of pharmacy technicians and interns. Provides medication preparation and distribution with a high degree of accuracy and under strict time-constraints. Develops safe, rational and cost-effective medication care plans to improve patient outcomes. Manages drug therapy and utilization across the continuum of care and monitors medication therapy outcomes. Educates patients and providers. Engages in professional activities, which improve the public's health awareness.
REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE Education:
  • Requires Bachelor's degree in Pharmacy.
  • Pharm.D. or other related advanced degree is desirable.
  • Maintenance of 30 continuing education credits every two years as required by the State Board of Pharmacy.

Experience:
  • Previous related experience or hospital residency is desirable.
  • Oncology or Infusion Experience is preferred.

Licensure:
  • A current State of Michigan Pharmacist license or eligibility for licensure.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES MAY INCLUDE
1. Review medication orders to determine appropriateness of therapy; including, but not limited to: dosage form, dose, and route of administration, schedule, starting time and duration.
2. Monitors patient medication regimens for possible allergies, drug interactions, nutrient-drug interactions, contraindications and potential adverse drug reactions.
3. Individualizes medication therapy based on patient variables including, but not limited to, renal function, age, weight, laboratory values and pharmacokinetic parameters.
4. Manages drug therapy and utilization across the continuum of care; documents clinical interventions, participates in QA monitors, drug utilization review, pharmacoecomomic analysis, and monitors medication therapy outcomes.
5. Performs selective patient medication histories and counseling; provides education materials as required.
6. Consults with and advises physicians and other health care professionals in the development of safe, rational and cost-effective patient medication care plans.
7. Responds promptly and thoroughly to requests from healthcare personnel for patient-specific pharmacotherapeutic information.
8. Directs and monitors pharmacy technicians, interns, aides, volunteers and students in preparation, packaging, labeling and distribution of medication.
9. Directs, monitors and verifies the compounding of injectable medications including epidural infusions, irrigation, small and large volume parenterals, hyper alimentation and cytotoxic agents performed by pharmacy technicians.
10. Compounds and dispenses medications and injectables recognizing that compounding and/or dispensing errors may result in significant patient morbidity and mortality. Competency must be demonstrated before compounding or checking compounded products. Adheres to USP 797/USP 800 Guidelines.
11. Participates in hospital emergency codes responses. Provides and prepares emergency medications via emergency carts, cardiac kits and EMS/Ambulance kits.
12. Coordinates pharmaceutical services for specified hospital units. Problem-solves, communicates with, and serves as a liaison between these units and the pharmacy department.
13. Initiates, directs, coordinates and performs specific departmental projects. 14. Adheres to and enforces Pharmacy & Therapeutics and departmental policies and procedures.
15. Participates in teaching programs affiliated with the Department of Pharmacy and training programs for nursing, physicians, pharmacists and other allied health professionals.
16. Performs duties in compliance with all federal, state and local regulatory requirements.
17. Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution
18. Maintains the confidentiality of information acquired pertaining to patient, physicians, associates, and visitors to St. Joseph Mercy Health System. Discusses patient and hospital information only among appropriate personnel in appropriately private places.
19. Behaves in accordance with the Mission, Vision and Values of SJMHS.
20. Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.
