JOB SUMMARY: The Triage Registered Nurse (RN) will be the first line for phone calls coming in from patients and families or field staff. The Triage RN will be able to answer questions regarding hospice services, re-fill prescriptions, assist with ordering supplies and DME.QualificationsREQUIRED EDUCATION & EXPERIENCE:Must be at least 18 years of ageMust have a high school diploma or equivalentMust be a Registered Nurse with knowledge of hospice, telephone triage preferredMust have an active RN license in good standing with the stateREQUIRED KNOWLEDGE, SKILLS & ABILITIES:Must show aptitude of computer data entry and the use of current software systems, such as word processing, spreadsheets and projectsMust possess the ability to deal with stressful situations, as well as grieving familiesMust be able to take patient/family phone calls to determine needs of the callerMust demonstrate the ability to provide professional and courteous interactions with patients, their families and staff under sensitive situationsOTHER REQUIREMENTS:Willingness to maintain comprehensive working knowledge regarding information systems and applicable software programsMust be dependable and flexible in work hoursESSENTIAL JOB FUNCTIONS:Follow up on phone calls by either directing the phone call to the DPCS, RN Clinical Supervisor/Manager or RN Case Manager and/or taking care of the need immediately per scope of practiceDispatch field nurses and other appropriate staff to patients residence in a timely mannerRe-fill prescriptions, order supplies and DME, and assist in any other coordination needed by the patients and familiesDocument calls, conversations and outcomes of call in software systemReceive start-of-shift report and give end-of-shift report in accordance with the location workflowWORK ENVIRONMENT:Must comply with company policies, procedures and processes in addition to Hospice and Homecare rules and regulationsMust comply with harassment and discrimination policiesMust foster good working relationshipsMust be able to speak and write in EnglishMust be a licensed driver with licensed automobile that is insured in accordance with organizations/state requirements and is in good working orderMust be willing to submit to a criminal background check as well as a drug/alcohol screening upon hire and periodically thereafterPHYSICAL ENVIRONMENT:Must be able to see and hear with/without accommodationsRequires consistent wrist movementRequires sitting for prolonged periodsMust be able to bend, stoop, kneel and climbMust be able to lift up to 20 lbs. without assistance and up to 50lbs.
