Hospice of the South Coast is currently hiring Part Time Compassionate On-Call Licensed Vocational Nurses (LVNs) to provide patient care in and around the San Diego Area!
Wednesdays & Thursdays 12a-8a
Saturdays & Sundays 4p-12a
The On-Call LVN is responsible for identifying and coordinating patient/family needs and for providing preventative and supportive care to the hospice patient and family. The On-Call LVN utilizes assessment, intervention, and teaching skills to maximize patient comfort and to enhance the quality of life. The On-Call LVN assumes on-call coverage for the designated teams and functions as an Interdisciplinary (IDT) team member to provide hospice care, support , and interventions after hours.

- Provides emergency, PRN, and routine visits as necessary or scheduled.
- Assumes primary responsibility for hospice patients during on-call hours.
- Provides ongoing assessments of physical, psychological, and social and spiritual needs of the hospice patient and family and initiates appropriate interventions.
- Reviews clinical referral information and responds to requests and inquiries as appropriate Collaborates with Administrator On-Call regarding the eligibility and appropriateness of client for hospice services.
- Assesses and evaluates patient's status by: following the plan of care, regularly reevaluating patient and family/caregiver needs, participating in revising the plan of care as necessary based on patient status and needs.
- Graduate of an Accredited Practical Nurse or Vocational Nurse Program.
- Holds current unencumbered license in the state of practice as an Licensed Practical Nurse.
- Minimum two (2) years' experience as LVN in Community Health, Medical surgical/Acute care setting preferred.
- One (1) year of hospice experience preferred but not required.
- Must be computer proficient in typing and various programs, including background in EMR.
- Possess and maintains current CPR certification if required by state.
$30.00 -$35.00 per hour (Average Pay Range). Pay will be determined based on experience.
