Responsibilities:
The functional evaluation of the client and the planning and utilization of a program of
purposeful activities to develop or maintain adaptive skills, designed to achieve maximal
physical and mental functioning of the client in his or her daily life tasks. Such treatment
program shall be rendered on the prescription or referral of a physician or nurse
practitioner.
- Responds to requests for service in a timely manner.
- Consults and coordinates care with clients, clients’ authorized practitioner and appropriate members of the agency.
- Establishes and updates a Plan of Care including treatments and goals.
- Instructs and supervises clients and other health care personnel in various manual and creative activities to promote increased independence in the client’s activities of daily living.
- Documents client care accurately and timely on appropriate forms per agency policy.
- Submits completed documentation in a timely manner in accordance with agency policy.
- Make recommendations to other health care professionals for referrals when
- appropriate.
- Updates knowledge and skills by attending in-services, case conferences and
- continuing education programs and informs agency of same.
- Respects clients’ rights and maintains confidentiality.
- Supervises Occupational Therapy Assistants.
- The following tasks are also considered essential functions of the position:
- Lifting unspecified weights, depending on assignment requirements.
- Bending and stooping to prepare equipment and materials for treating clients or managing the client’s environment.
- Transferring, turning and positioning clients and assisting clients in walking and moving from place to place.
Qualifications:
- Graduate with a degree in Occupational Therapy from an accredited institution.
- Currently licensed as a Occupational Therapist in state where therapist practices.
- A minimum of one year’s experience as a Licensed Physical Therapist in a supervised setting within the past three (3) years.
