Essential Functions:
- Comply with state practice act and/or American Occupational Therapy Association guidelines
- Coordinate care and relay information to Physicians, patients and family members along with the Home Health & Hospice interdisciplinary group in a timely and effective manner
- Deliver Occupational Therapy modalities appropriate to patients clinical status and needs
- Assess the patient’s need for durable medical equipment as well as other adaptive therapy
- Assess patients pain and utilize pain management that includes but is not limited to Occupational Therapy
- Provides, modifies or discontinues Occupational Therapy modalities based upon ongoing assessment/reassessment of patients clinical status and in accordance with physicians orders
- Participates in interdisciplinary team conferences in accordance with Agency policy
Qualifications:
- Must be a graduate of an approved/accredited occupational therapy program
- Must have a current license to practice physical therapy in state of employment
- Current driver's license and vehicle insurance, and access to a dependable vehicle
- Current BCLS certification
- Current health certificate/physical examination and TB testing
- Prior Home Health Experience preferred but not required
- Ability to pass criminal background check, drug/alcohol screening, and OIG screening

Salary: $57.00 - $82.00 per hour
