Job Description
Qualifications
- Master’s Degree in Occupational Therapy
- Current Occupational Therapist License in Illinois
- Early Intervention Credential (or process of obtaining)
- Experience as Occupational Therapist with children (birth to 3 years old) or experience with adaptive development; adaptive behavior, restoration, and play; and sensory, motor, and postural development.
Responsibilities
- Provide one on one service to children birth or 3 in their natural settings, such as their home and/or daycare
- Perform evaluation/assessment, six month and discharge summary reports
- Provide family home education program
- Follow IFSP (Individual Family Service Plan) goals, tracks and monitor progress
- Create a curriculum based on meeting each child’s goal
- Maintain and update client record as required to assure compliance with company policy, State and Federal Regulations and professional standards of practice
- Communicating with Early Intervention team to ensure quality of care
