Job Overview:
Far Government, is looking for a qualified Occupational Therapist, to provide Occupational Physical therapy services on site at St. Stephens Indian School.
Job Details:

• Job Title:Occupational Physical Therapist
• Location: Stephens Indian School, which is located at 128 Mission Rd., St. Stephens, WY 82524.
• Schedule: Part-Time.

Services shall be provided 2 times a week for 1.5 hours a day for 40 weeks throughout the school year and coordinated with the school. Service hours may increase or decrease based on student evaluations, and will be scheduled and coordinated on an ongoing basis.
Requirements & Skills:

• Must have a Master's Degree in Occupational Therapy
• Must have at least 5 years of experience, preferably with Native American students
• Must be certified by National Board for Certification in Occupational Therapy (preferred)
• Must be Licensed in the state of Wyoming

Specific Tasks, Duties & Responsibilities:
The Candidate shall provide Occupational Therapy services as follows:

• Review student's medical records, physician referral, child study team referral, parent request and/or school psychologist evaluation recommendation for further testing;
• Perform evaluation procedures to establish performance guidelines;
• Provide formal written comprehensive test results of student assessment 30 days from date of evaluation;
• Plan, prepare, and establish appropriate written Individual Education Plan (IEP) goals and objectives;
• Attend student's scheduled IEP meeting ;
• Administer and implement physical therapy/occupational therapy program ;
• Re-assess and revise program, as necessary, as status of student changes;
• Instruct and teach students, families and teachers as indicated on IEP;
• Make referrals to other health professionals and integrate physical/occupational therapy program with other aspects of health care;
• Confer with physician and other health practitioners to obtain information and make recommendations and revisions in treatment program ;
• Supervise and train education technicians and/or certified teachers in occupational/physical therapy assistance to be carried out when the occupational/Physical Therapist is not at the school;
• Provide weekly on site services during the school year; as warranted by student need;
• Provide a progress report for individual students' to coincide with the school's grading periods.
