Clinical Documentation Specialist/Clinical Documentation Nurse:
The Clinical Documentation Specialist (CDS) serves as an advisor and expert resource for Overlake Clinic providers to improve the accuracy of Clinical Documentation to support patient complexity and acuity, risk profiles and professional fee billing. The CDS primarily assists providers in capturing the Clinical Documentation needed to accurately reflect patient acuity. Focuses on the recapture or identification of chronic conditions reflected in Hierarchical Condition Categories (HCCs). Complete pre-visit chart reviews for scheduled patients to identify documentation opportunities for the provider to recapture previously documented HCCs diagnoses or new HCCs "suspect conditions" not previously captured that are identified by reviewing past medical records. Coordinate with colleagues on education and training geared toward improving Clinical Documentation.
Minimum Qualifications:
Education: Minimum of an Associate's degree is required. Equivalent combination of education and experience may be considered in lieu of degree.
Professional Licensure/Certification/Registration: CCS-P, CPC, CRC, or CCS required. Registered Nurse or Licensed Practical Nurse preferred.
Experience: Three years experience in physician professional coding in an inpatient or outpatient setting required. Clinical experience preferred.
Special Training/Skills: Knowledge of care delivery documentation systems and related medical records documents required. Strong communication and critical thinking skills, with ability to communicate effectively with all levels of the organization. Strong problem solving and investigative skills. Proficient with Excel, Word, and Power Point. Ability to create relevant and effective education materials.
Clinical Documentation Nurse
Minimum Qualifications:
Education: Graduate from an accredited school of nursing required. Bachelor's degree preferred.
Experience: Three years related clinical experience in an acute care setting required. Experience interacting with physicians required. ICD-10 coding and DRG experience preferred.
Professional Licensure/Certification/Registration: Requires current licensure as a Registered Nurse in the state of Washington. Must obtain CCS-P, CPC, CRC, or CCS certification within twelve months of hire date.
Special Training/Skills: Knowledge of care delivery documentation systems and related medical records documents required. Strong communication and critical thinking skills, with ability to communicate effectively with all levels of the organization. Strong problem solving and investigative skills. Proficient with Excel, Word, and Power Point. Ability to create relevant and effective education materials.
