The Nutritionist will perform individual counseling of patients referred by the medical personnel. Performs job duties in compliance with systems and job specific standards. Essential Job Duties and Responsibilities: • Provides nutrition counseling to patients referred by clinicians • Performs initial nutrition history on each patient • Develops and implements a care plan based on the assessment of nutritional and health needs • Provides instructions on a normal or special diet • Schedules follow-up appointments and evaluates the reaching of goals • Provides nutrition counseling for diabetic patients as part of diabetic care plan • Provides nutritional education materials to patients to support counseling efforts • Makes referrals to other health care providers and community services as appropriate • Counsels individuals and families on nutrition and diet based on the particular needs of the individual or family as determined by their nutritional and health risk • Attends staff meetings and in-service training • Provides consultation into the development of program policies and procedures • Provides direction and executes health education programs as needed • Must demonstrate knowledge and skill necessary to provide care appropriate to the age of the patients served Minimum Education Requirement : Bachelor’s Degree Related Work Experience : 2-4 years Required Licensure: RD/ RDN or CDN with a valid CPR card Specialized Skills & Knowledge: • Ability to communicate in English, both orally and in writing • Knowledge of nutrition principles as well as knowledge of nutrition during pregnancy and lactation, as well as infant and child feeding practices • Competence in nutrition assessment counseling
