About the Position:

Supervisory Duties:
  • Develop, update, and oversee medication administration procedures and policies, in conjunction with the Medical Director. Ensure residential counseling staff are provided adequate training and oversee adherence to medication administration policies and procedures.
  • In collaboration with the Residential leadership team and Medical Director, develop, update and oversee policy and procedures for the medical care of residential parents and their children.
  • Participate as a senior member of the Residential leadership team in program planning and oversight.
  • Supervise nursing and medical case management staff.
  • Participate on Residential referral team to ensure medical review and assessment of need for referrals and coordinate/communicate relevant medical care needs for new admissions
  • Provide ongoing education and information to staff related to health care of clients, Blood–borne pathogens, labor-support, infant and young child health and safety concerns, and medication administration.
  • Oversee WIC clinic, Pediatric Clinic and in-residence services provided by Community Health Center – Burlington. Communicate and collaborate with community health care providers and Vermont Dept. of Health as indicated.
  • Ensure responsible fiscal management through such practices as careful management of existing stock and medical supplies.
Client Services:
  • Ensure timely communication and planning for all health and medical related information to Residential staff for new admissions.
  • Obtain admission health histories and develop nutrition, childbirth, and general health plans with each resident.
  • Assess acute medical needs of clients in the program and document medical/nursing assessment and intervention with residents in nursing charts.
  • Connects clients to community healthcare providers for preventative and acute care needs.
  • Oversee the maintenance and strict accuracy of the Medication Administrative Record (MAR), including documentation of daily medication dispensing, as well as timely recording of all medication changes and additions.
  • Administer client medications.
  • Plan and implement a comprehensive prevention-focused health curriculum and educational opportunities for clients including on topics such as prenatal care, childbirth education, preventative health care, wellness, STI’s, contraception and family planning, infant care and nutrition, developmental and cognitive growth of children, and supporting clients to gain skills in advocating for self and child in health care.
  • Assess for special client health/medical needs and facilitate referrals for specialty care as indicated.
  • Provide health advocacy for clients and support them in the development of self-advocacy skills.
  • Provide transportation and accompany clients to medical and dental appointments, assist clients with information integration, and incorporate new medical information into existing client treatment planning. 
  • Document medical/nursing assessment and interventions with residents in nursing charts.
  • Attend status/provider meetings as indicated.
  • Coordinate medical discharge planning needs for clients including appropriate community referrals and attendance at discharge planning meetings.
  • Respond to the medical needs of clients in a 24/7 facility, providing triage or instruction to residential staff as needed.
  • This position starts at $80K - 84K/annually
  • Other duties as assigned.

What We Look For: 

  • Must be a Licensed Registered Nurse with the State of Vermont.
  • Bachelor's Degree in a related field required.
  • Experience working with women and children in hospital and community settings, use of women centered recovery model, family centered nursing care, and experience in psychotropic medications.
  • Ability to work and collaborate with multiple disciplines in providing nursing care to the population served in the Residential program.
  • Education should be an ongoing process. Ability to adhere to continuing education requirement to maintain licensure.
  • Willing to travel and present information to community in various settings to promote and initiate support for Lund Family Center.
  • Supervisory experience preferred.
  • Ability to perform the essential functions of the position either with or without reasonable accommodation.
  • Ability to maintain a valid VT Driver’s License and access to reliable transportation. Occasional statewide travel may be required.
  • Upon conditional offer of employment, must be able to successfully pass a background check.
  • Regular attendance is necessary and essential to meeting the expectations of the job.
