Full-Time Position
Position Summary
This position will focus on all of Hawaii Care Choice’s contracted long-term care facilities to improve, facilitate and ensure continued excellence of service. Uses and established work systems and processes to enable quality of care and quality of life while ensuring ongoing regulatory compliance with all applicable laws and professional standards of practice. Ensures effective communication of needs and issues to team and fosters positive change, professional/program growth and teamwork. Promotes Hawaii Care Choices mission, vision and values to develop positive relationships with the community and all partners.
Duties & Responsibilities
- Responsible for serving as an advocate for residents rights in long term facilities.
- Responsible for establishing strong and meaningful relationships.
- Maintains and ensures all medical records are obtained from all contracted facilities and that they are updated and current.
- Aides with daily administrative support and outreach.
- Understands and complies with conditions of participation.
- Coordinates all Medical Director visits with facilities.
- Practice effective communication and interpersonal skills to enhance positive teamwork and accomplish goals/tasks.
- Coordinates bereavement services for patients in care facilities.
- Assists in educating and setting up educational activities.
- Demonstrate initiative and work as a team player.
- Collaborate with team and makes necessary recommendations to ensure needs are met and in line with the overall strategic initiatives of the organization.
- Promote and market Hawai`i Care Choices.
- Establish working relationships with all company support departments.

- Excellent written and verbal communication skills as well as the ability to communicate effectively with all levels of individuals.
- Basic computer knowledge of MS applications such as Word, Excel, Outlook.
- Working knowledge of departmental and organizational policies, procedures, and operations.
Education & Experience Requirements
- High school diploma or higher required.
- 2yrs Healthcare environment experience is preferred.
Hours
- Primarily Monday through Friday, 8:00 a.m. to 4:30 p.m., with a half-hour lunch break.
- Duties may occasionally require after hour work including weekends as needed.
Job Type: Full-time
Pay: $25.00 - $29.00 per hour
Expected hours: 40 per week
