Summary of Duties: Assists Nursing Staff in caring for Hospice patients at the inpatient facility; performs routine duties and follows instructions given by Nursing staff; exercises care in handling patients; reports any noticeable changes in condition or behavior to Registered Nurse.
Essential Functions of the Job
Assists in admitting, transferring or discharging patients; stores and records patient’s clothing and valuables; completes proper forms.
Assists with serving food trays and nourishments; records intakes as required.
Reports to Registered Nurse poor intakes or refusal of nourishments.
Reports and documents all observed falls and unusual occurrences by completing an incident report.
Provides for measures which provide for the safety of patients (i.e. side rails, fall risk-protocol, bed exit alarm, soft vest/posey restraints).
Maintains an attractive and comfortable environment for patients with special consideration to the cleanliness of rooms, ventilation and lighting.
Adheres to and effectively communicates Hospice’s philosophy, goals, objectives and policies to patients/families, physicians and staff.
Ensures that all patient information is held/handled in a strictly confidential manner.
Maintains patient personal hygiene needs; gives baths (i.e. bed, shower); assists with overall grooming if patient unable to do so by themselves.
Gives shampoos.
May initiate oxygen.
Collects specimens as necessary (i.e. sputum, urine, feces).
Assists with feeding.
Gives oral- care, back- care, and peri-care.
Performs foley catheter care, and post-mortem care.
Assists in transferring patient to and from bed; to bed; to wheelchair; to commode, etc., turns and changes patient’s position.
Assists and accompanies patients with ambulation.
Gives ROM exercises when appropriate.
Makes bed and keeps immediate environment safe and clean.
Gives selected enemas under direction of Registered Nurse.
Carries out isolation techniques in patient care when appropriate.
Reports any change in patient's mental or physical condition to Registered Nurse.
Assists patient in dressing and applying assistive devices when necessary.
Maintains responsibility to ensure current CNA certification and current CPR.
Prepares and participates in regulatory/accrediting surveys (i.e. DOH/JCAHO).
PERFORMS ALL TASKS AND RELATED DUTIES AS ASSIGNED
Educational Requirements: High School Graduate or equivalent. Certificate of completion of a State of Hawaii certified Nurse’s Aide course (100 hours). State of Hawaii Nurse Aide Certification, preferred.
Experience: Six months Nurses’ Aide experience or equivalent clinical experience. Acute care experience preferred. Will consider Skilled Nursing Facility and Home Health Aide experience.
Skills Required: Dexterity in handling equipment and instruments. Ability to adapt to changing situations on short notice and with ease. Able to express self clearly and effectively in oral and written form. Effective interpersonal and communication skills. Current CPR certification.
