Under the direct supervision of a registered nurse, the Certified Nursing Aide performs patient care activities and other task as assigned to assist with primary care in the home and the clinic. He/she also assists therapist and adult day health staff in providing optimal care for the participants.
Responsibilities
Provides and documents patient care under the direction of a registered nurse and as outlined in the plan of care
Participates in quality initiatives resulting from evidence-based literature and/or unit-based metrics
Creates and maintains an environment that protects participants from harm
Applies PACE Center service excellence best practices in approaching participants, visitors, physicians, and colleagues
Communicates and collaborates with the interdisciplinary team and/or other departments in a consistent and timely manner
Handles conflict resolution opportunities in an appropriate, timely, and consistent manner
Contributes to safe and efficient admission and transport during arrival and departure from the PACE Center
Uses resources (e.g., time, people, supplies, etc.) cost effectively
Engages in own professional development as evidenced by self appraisal, and earning academic credits and/or attendance at educational programs
Identifies personal contributions to PACE Center cascading goals
Participates in the planning and implementation of PACE Center activities under the direction of the Recreation Therapist
Provides personal care assistance to participants in their homes
Other duties as assigned
Qualifications
Required Education: Successful completion of a state approved Nurse Aide education program; or, if currently enrolled in a nursing degree program, must have completed at least one (1) clinical course
Preferred Education: High School Diploma / GED
Required Experience: One (1) year of experience working with the frail or elderly
Preferred Experience: Two (2) or more years of long term care experience
Required Certifications and Licensures: Must be certified as a Nurse Aide by the Department of Health Professions of Virginia. Valid Virginia Driver's License
