The Archdiocese of Chicago is the third largest diocese in the United States and serves more than 2.2 million Catholics in The Archdiocese, pastored by Cardinal Blase J. Specific Duties and Responsibilities:
1. Provide health care and services for students as follows:
• Provide health counseling.
• Develop, implement, monitor, and evaluate individual nursing care plans.
• Meet with parents of children with health concerns before the start of the school
year to make sure their child's needs are being met at school.
• Delegate nursing care as per nursing care plan.
• Supervise the administration of all medications.
• Report incidents of suspected child abuse and neglect.
• Monitor attendance and notify administration with increased absences.
• Participate on the SST team when health related students are evaluated.
2. Be responsible for Archdiocese, State, and Illinois Department of Public Health requirements as follows:
• Arrange for vision and hearing screening in compliance with Illinois Department of public health requirements. Recheck students who do not fall within the
reference range, make referrals, and provide follow-up as needed.
• Collect and copy medical and emergency contact forms and provide a copy to each teacher and one for the emergency folder.
• Evaluate health information forms and complete a list of health concerns for each teacher that comes into contact with the student
• Monitor for State compliance: all immunization and physical, dental, and eye exam records for preschool, kindergarten, 6th grade and any student participating in interscholastic sports in 5th-8th grades. Report compliance to the State of Illinois.
3. Assume responsibility for health records as follows:
• Ensure that student health records are current and accurate.
• Ensure that health records are maintained in a confidential file.
• Collect and evaluate Camp Duncan forms and create a medication administration record for staff.
4. Arrange for emergency care for accidents or injuries at school as follows:
• Follow procedures for first aid as stated in the Health Office Manual.
• Contact parents of students
• Advise staff and lunchroom volunteers of proper procedures to follow in the event of severe illness or accident. Provide direction and training for use of 911
services.
• Provide and maintain basic first aid kits for recess staff and to be taken on field trips.
• Collaborate with staff on procedures to ensure the health and well-being of students in the event of a lockdown or weather emergency.
• Monthly checks of AED's and ordering new supplies as needed.
5. Be responsible for infectious disease control as follows:
• Provide training and in-service related to infectious disease control including blood-borne pathogens and Universal precautions.
6. Participate in health education as follows:
• Serve as a health resource person to St. Joseph faculty and staff, students, and parents.
• Assist in conducting units on hygiene, hand washing, nutrition, and other health topics as requested.
• Assist with education of staff through in-services on CPR, Universal Precautions, Blood Borne Pathogens, Epi Pen use, Diabetes Care and Asthma.
• Serve as a health resource for individuals and community groups.
• Attend the mother-daughter tea as staff liaison and support parents on providing a speaker.
7. Assume responsibility for assisting families as follows:
• Serve as a contact person between the home and school on health problems.
• Make sure accommodations, in collaboration with faculty are in place whether through a 504 or nursing care plan for medically needy students.
• Assist parents in providing for student's health care and assist them in accessing community services.
• Interpret school health policies, procedures and practice to families.
• Provide health related information through school newsletters and letters to parents.
